<trans-abstract xml:lang="en">
<title>BACKGROUND</title><p>BACKGROUND: Locally advanced stage cT3a renal cell carcinoma is characterized by invasion of paranephric cellular tissue, renal sinus or main venous structures and is diagnosed in 20–30 % of cases. While radical nephrectomy remains the “gold standard”, data accumulated in recent years show comparable oncological safety of kidney resection for stage cT3a disease. Patients with solitary kidney remain the most controversial group.</p>
<title>AIM</title><p>AIM: To evaluate immediate, perioperative and early functional results of resection of single-functioning kidney in patients with locally advanced stage cT3a cancer of renal parenchyma.</p>
<title>METHODS</title><p>METHODS: The retrospective-prospective study included patients with stage cT3a cancer of renal parenchyma who underwent surgery at the Institute of Human Urology and Reproductive Health of the Sechenov First Moscow State Medical University between January 2014 and October 2025. All patients underwent kidney resection. In total, 20 (2.8 %) of 703 patients had single-functioning kidney. Clinical, anatomical, perioperative, early functional characteristics, as well as frequency of achieving renal trifecta and pentafecta, in these patients and 356 patients who underwent organ-saving treatment and have two kidneys were compared.</p>
<title>RESULTS</title><p>RESULTS: Patients with solitary kidney had higher creatinine level (104.50 µmol/L versus 90.45 µmol/L; p = 0.0015), lower glomerular filtration rate (56.37 mL/min/1.73 m² versus 71.74 mL/min/1.73 m²; p &lt;0.0001) and hemoglobin level (129 g/L versus 140 g/L; p = 0.0171). Operative time was shorter in patients with solitary kidney compared to patients with two kidneys (95 min versus 120 min; p = 0.0220). No differences were found in ischemia time, blood loss volume and duration of hospitalization. Negative surgical margin (R0) rate in patients with solitary kidney was lower (55.0 % versus 90.0 %; p = 0.0055). Frequencies of achieving trifecta (63.6 % versus 77.7 %; p = 0.2790) and pentafecta (45.5 % versus 35.7 %; p = 0.5357) did not show significant differences.</p>
<title>CONCLUSION</title><p>CONCLUSION: Resection of single-functioning kidney in locally advanced stage cT3a cancer of renal parenchyma is technically feasible and is accompanied by perioperative profile comparable to profile of patients with two kidneys except for higher risk of positive surgical margin. Imperative character of the intervention requires accurate planning of resection volume and personalization of surgical tactics.</p>
</trans-abstract><kwd-group xml:lang="ru"><kwd>рак паренхимы почки</kwd><kwd>cT3a стадия</kwd><kwd>резекция почки</kwd><kwd>единственная функционирующая почка</kwd><kwd>хроническая болезнь почек</kwd></kwd-group><kwd-group xml:lang="en"><kwd>renal cell carcinoma</kwd><kwd>stage cT3a</kwd><kwd>partial nephrectomy</kwd><kwd>solitary functioning kidney</kwd><kwd>chronic kidney disease</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">World Cancer Research Fund.
