Тулиевая вапорезекция предстательной железы (ThuVaRP) представляет собой достаточно безопасную и эффективную методику лечения доброкачественной гиперплазии предстательной железы (ДГПЖ). Она не требует длительного периода обучения и дополнительного дорогостоящего оборудования. Ограниченное число исследований по оценке результатов ThuVaRP обусловливает актуальность настоящего исследования.Материалы и методы. В проспективное исследование включено 405 мужчин с ДГПЖ, которым была выполнена ThuVaRP в период с 2018 по 2024 год с использованием тулиевого волоконного лазера («Уролаз» I поколения, НТО «ИРЭ-Полюс», Фрязино, Россия). При проведении операции одновременно происходило и резецирование, и испарение ткани предстательной железы до выделения ее капсулы.Результаты. Возраст пациентов варьировал от 49 до 93 лет (медиана – 69 лет), объем предстательной железы – от 35 до 198 см3 (медиана – 76 см3), максимальная скорость мочеиспускания – от 0 до 11,2 мл/с (медиана – 7,3 мл/с), объем остаточной мочи – от 35 до 220 мл (медиана – 110 мл). У 85 (21,0%) пациентов имело место наличие надлобкового дренажа. Продолжительность операции составляла от 49 до 93 мин. (медиана – 69 мин.), расчетная масса удаленной ткани – от 24 до 173 г (медиана – 55 г), снижение гемоглобина после операции – от 3 до 15 г/л (медиана – 6 г/л), длительность катетеризации – от 3 до 8 дней (медиана – 5 дней), пребывание в больнице после операции – от 4 до 15 дней (медиана – 6 дней). В ранние сроки после операции отмечены следующие осложнения: кровотечение – у 6 (1,5%) пациентов, инфекция мочевыводящих путей – у 58 (14,3%), острая задержка мочи – у 19 (4,7%), транзиторное ургентное недержание мочи – у 43 (10,6%). В поздние сроки встречались следующие осложнения: стеноз шейки мочевого пузыря – у 7 (1,7%) пациентов, стриктура уретры – у 2 (0,5%), стрессовое недержание мочи – у 15 (3,7%), рецидив ДГПЖ – у 13 (3,2%). Сроки послеоперационного наблюдения пациентов варьировали от 3 до 70 месяцев (медиана – 32 месяца). За время наблюдения отмечено достоверное улучшение показателей мочеиспускания после операции относительно предоперационных данных.Заключение. ThuVARP может быть успешно применена для лечения ДГПЖ вне зависимости от размеров предстательной железы, в том числе у пациентов, получающих антикоагулянтную и/или антиагрегантную терапию. В силу своих свойств она может быть рассмотрена как альтернатива методам трансуретральной резекции и лазерной энуклеации предстательной железы.</p></abstract><trans-abstract xml:lang="en"><p>Introduction. Thulium vaporesection of the prostate (ThuVaRP) is a relatively safe and effective surgical procedure for benign prostatic hyperplasia (BPH). It does not require a long training period or additional expensive equipment. The limited number of studies evaluating the results of ThuVaRP makes this study relevant.Materials and Methods. The prospective study included 405 men with BPH who underwent ThuVaRP between 2018 and 2024 using a thulium fiber laser (Urolaz 1st generation, NTO IRE-Polus, Fryazino, Russia). During the operation, both resection and evaporation of prostate tissue occurred simultaneously to isolate its capsule.Results. The patients’ age ranged from 49 to 93 years (median 69 years), prostate volume from 35 to 198 cm3 (median 76 cm3), maximum urination rate from 0 to 11.2 ml/s (median 7.3 ml/s), residual urine volume from 35 to 220 ml (median 110 ml). 85 (21.0%) patients had suprapubic cystostomy drainage. The duration of the operation ranged from 49 to 93 minutes (median – 69 min), estimated mass of removed tissue – from 24 to 173 g (median – 55 g), decrease in hemoglobin after surgery – from 3 to 15 g/l (median – 6 g/l), duration of catheterization – from 3 to 8 days (median – 5 days), hospital stay after surgery – from 4 to 15 days (median – 6 days). The following complications were noted in the early postoperative period: bleeding – in 6 patients (1.5%), urinary tract infection – in 58 (14.3%), acute urinary retention – in 19 (4.7%), transitory urgent urinary incontinence – in 43 (10.6%). The following complications were observed in the late stages: bladder neck stenosis in 7 patients (1.7%), urethral stricture in 2 patients (0.5%), stress urinary incontinence in 15 patients (3.7%), and recurrent BPH in 13 patients (3.2%). The postoperative follow-up period varied from 3 to 70 months (median 32 months). During the entire follow-up period, a significant improvement in key urination parameters was observed after surgery compared to preoperative data.Conclusion. ThuVARP can be successfully used to treat BPH regardless of the size of the prostate gland, including in patients receiving anticoagulant and/or antiplatelet therapy.
