Доброкачественные экстратестикулярные образования, в том числе липомы семенного канатика, часто являются случайными находками при пальпации мошонки. Нередки случаи выявления липом как при выполнении герниопластики паховых грыж, так и при других оперативных вмешательствах в паховом канале либо в мошонке. Липомы семенного канатика могут сопровождать грыжу пахового канала, а в некоторых случаях и исходить самостоятельно.Некоторыми авторами были описаны случаи ошибочной диагностики огромных экстратестикулярных липом как паховых грыж, при этом предлагалось признать липому семенного канатика важной клинической единицей, необходимой при проведении тщательной урологической дифференциальной диагностики с целью избежания ненужных герниопластик.Определить правильные тактические и технические приемы, их последовательность, адекватность для каждого конкретного случая не всегда просто. Это связано с ограниченным временем для принятия решения и высокой эмоциональной нагрузкой, которая ложится на врача при обследовании и выборе тактики лечения в момент оперативного вмешательства. Этот факт побудил нас поделиться нашим клиническим наблюдением и предложить используемую нами пошаговую хирургическую тактику.

Benign extratestinal lesions, including spermatic cord lipomas, are often incidental findings on palpation of the scrotum. There are frequent cases of detection of lipomas both when performing hernioplasty of inguinal hernias, and during other surgical interventions in the inguinal canal or in the scrotum. Lipomas of the spermatic cord can accompany the hernia of the inguinal canal, and in some cases, and proceed independently.Some authors have described cases of erroneous diagnosis of huge extratesticular lipomas as inguinal hernias, while suggesting that the lipoma of the spermatic cord be recognized as an important clinical unit necessary in conducting a thorough urological differential diagnosis in order to avoid unnecessary hernioplasty. It is not always easy to determine the correct tactical and technical techniques, their sequence, adequacy for each specific case. This is due to the limited time for decision-making and the high emotional burden that the doctor bears when examining and deciding treatment tactics at the time of surgery. This fact prompted us to share our clinical observations and suggest step-by-step surgical tactics that we use.
